Step 1: If you haven't done so, please select your doctor

You may review our "Primary Care, Specialty Care, & Associates Profiles" brochure or you may click on Find A Doctor. If you require personal assistance, call (858) 675-3100 between 8:00 am & 4:30 pm, Monday - Friday. Infants & children can be seen in Family Practice as well as Pediatrics. Family Practice treats ALL ages while Internal Medicine treats those who are 18+ years of age.

Step 2: If you are required (e.g., HMO), please notify your Health Plan/Insurance of the doctor you selected. Call the toll-free number for Member Services on your insurance card. Please keep in mind that your Health Plan may have your doctor listed under Centre For Health Care or under Park Terrace Medical Associates. Same-day appointments are arranged based upon urgency and availability. If you must cancel an appointment, please try to notify the doctor's office at least 24 hours prior to the appointment.
